People: Rise extends the care team with practice coordinators who are often the first point of contact for patients—they answer patient calls and begin the process of data gathering and engaging patients to support a better care model. In addition, practice consultants work closely with physicians to analyze the current state of their practices and build more efficient processes and workflows. Consultants train support staff and physician extenders on incorporating improved processes into the practice and using existing and new technologies to manage internal and external information.
Process: A Rise-enabled practice can manage operational tasks more efficiently while handling processes directly related to care with consistency and reliability. In particular, RiseHealth team members contribute to increasing the effectiveness of pre-visit planning, the care visit and follow-through care.
Technology: The Rise Technology System (RTS) captures internal information, such as patient evaluations and scheduled appointments, and integrates it with external information such as ER admissions and prescription records from pharmacies. RTS organizes the data so physicians and support staff can make timely and accurate decisions that directly impact care decisions.
