Rise brings these three capabilities to life in three distinct phases of care delivery. These phases of care delivery are equally important regardless of whether the patient is healthy with symptoms of a cold or flu or is a medically complex patient with multiple chronic conditions and co-morbidities. All three phases of the care process are critical to achieving the three strategic goals described above.
Phase 1: Pre-visit Planning
RiseHealth patient coordinators are trained to engage with patients from their first call to collect information that helps primary care practices plan and schedule the right kind of visit. The pre-visit process begins when a patient first decides to seek care---it is critical that the patient come to see the primary care practice as their first stop if they have a health problem or concern. During that call, information from Rise Technology System (RTS) combined with evidence helps prepare the patient and the practice for a more complete and effective visit experience.
Phase 2: The Visit
With pre-visit work completed, the provider and the primary care practice are prepared for the visit, which allows the patient and physician more time to do what they are uniquely suited to do—discuss treatment options and create a unique care plan.
Phase 3: Care Planning and Follow-through
The Rise care team works with the provider to help patients follow a unique care plan. This may be as simple as helping a patient follow a diet or as complex as managing a discharge following hospitalization.
